Support Co-ordination

With many support services available through the NDIS scheme, it is important to find which specific support services a required for each individual. Support Co-ordination is a vital aspect of what we do and allows us to tailor the support services that we have available to best meet the requirements and needs for each of our clients. Over a period of time, support requirements may change whether a person is becoming more independent and is able to start performing more tasks on their own, or if difficulties arise and additional support services and tasks may need to be appointed, this is where a personal support co-ordinator will benefit.

The support coordinator will get to know you and your specific needs so that they can create a support plan based on the services that will best benefit you. They will explain how the NDIS system works, give you information about what services are available to them for their situation and discuss with you what services you would like to have allocated for you. The support coordinator will then arrange all the services agreed upon and will then connect you to the support staff and have everything arranged based on the discussed plan. Over time the coordinator will check back in to make sure everything is going well and to see if any updated services are required or if any changes in the support plan is necessary.
